The Survivor's Questionnaire
was created for survivors of cult trauma,
ritual abuse, satanic ritual abuse, spiritual
abuse, sexual abuse, child abuse etc. No
identifying information is shown; it is
statistical data for survivors of abuse of all
kinds, MPDs and dissociatives.

I
am not a Psychologist; I am an ordained certified Restoration Prayer
Minister and was an experienced mental health
nurse. We are offering Christian
Restoration Prayer Ministry
by phone or in person at Cross Walk Life, at your ministry or
church.
Restoration Prayer Ministry appointments are
available to those who send the application and commit to a minimum
of 6 three hour sessions. You may find that this is
the best option for you, especially if you do not
live in the western metro Pittsburgh area. Most clients prefer
to use their "unlimited long distance plan" or to
use their cell phone. (Keep in mind that we are in
Eastern Standard Time.) Our Toll Free lines
cannot be used for appointments by the hour.
